Banner Home Health is a four-star home care agency with a track record for quality and excellence and is the largest skilled home care agency. Our structure is set up to provide Home Health Nurses multiple avenues for support that aren't available at other agencies a field preceptor, an IT specialist, a dedicated scheduler, a nurse preceptor during orientation (available every day to help with documentation questions and challenges), an in-office team of QI specialists, telehealth resources, clinical manager and in office triage support. This structure is designed to assist field clinicians in managing their caseload appropriately and not feeling "alone" in the field! As a Home Health Registered Nurse, you will join the Banner Home Health Clinical Team providing exceptional, expert care in your community. You will enjoy an autonomous practice focusing on medical intervention within the patient's actual home environment. The Home Health space allows for the time to create one on one relationships with your patients and their caregivers and have some flexibility over your schedule that is not possible in the hospital setting. You will enjoy working in a sub-assigned geographic territory with a smaller travel distance than other valley agencies! You will enjoy an autonomous but supportive work environment. We offer mileage reimbursement and will provide you a company cell phone and a tablet for documentation in our electronic medical record system (Home Care Home Base).

• Saturday, Sunday, Monday
• 12 Hour Days
• On-Call Rotation

The care we provide focuses on assisting patients to achieve an optimum level of health and enhancing the quality of life in the home setting for patients. Our home care services are based on a physician's plan of treatment and designed to manage total care. We also educate patients and families to promote an understanding of the disease process, its prevention and treatment.

POSITION SUMMARY

This position assesses, plans, implements, evaluates and documents nursing care of patients in accordance with organizational policies and in accordance with standards of professional nursing practice utilizing the framework for professional nursing practice and development. This position is accountable for the quality of nursing services delivered by self or others who are under their direction. This position utilizes specialized knowledge, judgment, and nursing skills necessary to assess data and plan, provide and evaluate care appropriate to the physical and developmental age of assigned patients.

CORE FUNCTIONS

• Assess patient physical, psychological, social, spiritual, educational, developmental, cultural and discharge planning needs.
• Formulate a plan of care, including the discharge plan, utilizing assessment data and patient, family and health team input.
• Implement the plan of care through direct patient care, coordination, delegation and supervision of the activities of the health care team.
• Evaluate the patient's and family's response to care and teaching, and revise the plan of care as needed.
• Document assessment, planning, implementation and evaluation in the patient record.
• Provide care based on the best evidence available and may participate in research activities within clinical practice.
• Interacts with all levels of staff in a variety of departments, physicians, patients, families and external contacts, such as employees of other health care institutions, community providers and agencies, concerning the health care of the patient.

MINIMUM QUALIFICATIONS

Must possess a current, valid RN license in state of practice, temporary/interim RN license in state of practice, or compact RN licensure for current state of practice. BLS certification is required. Additional certification or continuing education may be required based on area of practice.

PREFERRED QUALIFICATIONS

Bachelor's degree preferred. Professional certification preferred. Additional related education and/or experience preferred.

$33.11 - $55.18 / hour
