Job Summary: The Nurse Clinician - Outpatient Parenteral Antimicrobial Therapy (OPAT) serves as an advanced nurse clinician specializing in the coordination, management, and monitoring of patients requiring intravenous antimicrobial therapy, high-risk oral antimicrobial regimens, Total Parenteral Nutrition (TPN), and vascular access device (VAD) support across the continuum of care. Provides comprehensive case management, discharge planning, patient education, and care coordination for patients transitioning from the acute care setting to home, infusion centers, skilled nursing facilities, dialysis centers, and other outpatient care environments. Collaborates closely with infectious disease specialists, physicians, advanced practice providers, pharmacists, case managers, home health agencies, infusion pharmacies, and other healthcare professionals to ensure safe, effective, evidence-based treatment and seamless transitions of care. Responsibilities include monitoring clinical progress, laboratory results, therapeutic drug levels, vascular access device status, and treatment adherence; identifying and addressing barriers to care; facilitating timely interventions; and promoting positive patient outcomes. Utilizing advanced clinical knowledge, evidence-based practice, and Infusion Nurses Society (INS) Standards of Practice, serves as a patient advocate, clinical resource, educator, and liaison for patients, families, caregivers, and interdisciplinary teams. The role supports quality improvement initiatives, regulatory compliance, research activities, staff education, and program development while maintaining accurate documentation and ensuring exceptional patient-centered care, safety, and customer service excellence. Through leadership, collaboration, and professional accountability promoting continuity of care, optimal treatment outcomes, and an enhanced patient experience throughout the OPAT treatment journey. Performs other duties as needed.

Education and Experience: Bachelor’s degree in nursing required. Master’s degree in nursing from preferred. Minimum three years of experience as an acute care nurse required, preferably with infusion care patients (e.g., chemotherapy, and/or TPN/VAD, GI patients).

Knowledge and Skills: Advanced knowledge about antimicrobial therapy, long term vascular access lines, INS recommendations, local, state, and federal regulations and accreditation. Ability to apply theoretical concepts to clinical practice and continually expands knowledge base and clinical skills with a focus in area of specialty and population served. Demonstrates advanced competencies in gastrointestinal clinical nursing. Ability to apply theoretical concepts to clinical practice and continually expand knowledge base and clinical skills Good understanding of data base management, able to enter data and extrapolate date. Able to: read; write legibly; speak in English (and Spanish preferred) with professional quality; use computer and software programs necessary to the position; troubleshoot and calibrate patient care equipment. Able to relate and communicate positively, effectively, and professionally with others; be assertive and consistent in following and/or enforcing policies; work calmly and respond courteously when under pressure; lead, supervise, teach, and collaborate; accept direction. Able to communicate effectively in English in person, in writing, and on the telephone; think critically; work independently with minimal supervision; perform basic math functions; manage multiple assignments effectively; work well under pressure; problem solve; organize and prioritize workload; recall information with accuracy; pay close attention to detail. Able to distinguish colors and smells as necessary for patient care; hear sufficiently for general conversation in person and on the telephone; identify and distinguish various sounds associated with the work place/patient care; see adequately to read computer screens, medical records, and written documents necessary to position; discern temperature variances through touch.

Licensures and Certifications: California Registered Nurse (RN) license required with specialty certifications preferred as a Certified Registered Nurse for Infusion (CRNI).
