divh2In Home Services Cna/h2pThe In Home Services CNA is responsible for providing routine care and assisting participants or clients with activities of daily living, restorative and supportive care. Physical attendance at an InnovAge facility and/or an InnovAge participant home is essential to performing the job duties./ppResponsibilities include:/pulliFollowing plan of care and assisting participants with activities of daily living such as bathing, dressing, grooming, hair care, mouth care, eating, hand and toileting as assigned or needed. Documents care provided in accordance with established policies, guidelines and applicable regulations/liliCarrying out, reporting and documenting care given in an effective, timely manner as observed by the RN and through periodic record reviews./liliParticipating in the care planning of participants through cooperation with the multidisciplinary teams./liliMonitoring participants health status by taking vital signs and weights (monthly)./liliMonitoring appetite in addition to looking for changes in functional ability and checking skin condition while assisting with toileting./liliMonitoring/assisting participants in a courteous manner with meals and providing assistance with feeding as necessary./liliAssisting participants with restorative and supportive care such as ambulating, positioning, transferring, skin care, catheters, fills oxygen and providing other aides as assigned or needed./liliMonitoring participants health status such as vital signs, weights, skin condition, appetite and changes in functional ability and reporting pertinent information to clinical supervisor in a timely manner./liliReporting any variance, accident or unusual occurrence to the appropriate personnel./liliAttending in-service and continuing education programs as scheduled and necessary./liliPerforming nursing assistant services to clients in their place of residence/liliAttending position related in-services. Attending all mandatory in-service programs as scheduled; minimally 12 hours/year/liliParticipating in QM activities as requested./liliCompleting appropriate visit records in a timely manner as per Agency policy/liliPerforming simple procedures as an extension of the therapy or nursing services as assigned./liliPerforming household services essential to health care in the home as assigned./liliDoes not accept assignments for a patient with special needs for which he/she has not received appropriate training./liliAdditional work functions and duties may be assigned/li/ulpRequired:/pulliCurrent and valid state issued Certified Nursing Assistant (CNA) license/liliCurrent CPR certification or the ability to obtain CPR certification./liliRequire personal transportation, current state issued drivers license, good driving record and auto insurance as required by law/liliA minimum of one year experience working the frail or elderly./liliHigh School Diploma / GED or relevant work experience in lieu of education./li/ulpPreferred:/pulliBi-lingual/li/ulh2Benefits/h2pInnovAge is dedicated to empowering seniors to live independently, allowing them to age in their own homes and communities safely.
