Responsibilities:

• Conduct assessments and develop individualized nutrition plans for residents, taking into account their unique needs, preferences, and dietary requirements

• Collaborate with the interdisciplinary care team to develop and implement care plans that promote optimal health outcomes

• Provide nutrition education and counseling to residents, families, and caregivers

• Monitor and assess the effectiveness of nutrition programs and make adjustments as needed

• Develop and implement innovative nutrition programs, ensuring compliance with regulatory requirements and company standards

• Maintain accurate and detailed records of resident nutrition plans and progress

Work Environment:
