Join Chapa-De as a Referral Coordinator and play a key role in helping patients access timely specialty care and telehealth services. Hourly Rate: $23.00 - $25.80 per hour

Duties:

• Coordinate and process medical referrals, prior authorizations, and specialty care appointments while ensuring timely follow-up and accurate documentation within the EHR system.
• Serve as a liaison between patients, providers, specialists, and insurance carriers to support seamless communication and continuity of care.
• Support Chapa-De's telehealth services by scheduling appointments, coordinating eConsults, preparing equipment, and assisting patients and providers during virtual visits.
• Monitor referral workflows, insurance portals, and referral statuses to ensure quality, compliance, and an exceptional patient experience throughout the referral process.

Qualifications:

• High School Diploma or equivalent.
• One year customer service experience in a healthcare environment using a medical EHR system with understanding of basic medical terminology.
• Prior experience answering phone calls and using computer-based applications to coordinate patient care.
• Familiarity with or prior use of Microsoft Office (ex: Outlook, Teams, Excel)
