Nurse (Case Management)

Summary Join the Indian Health Service and care for diverse American Indian and Alaska Native communities. In this role, you’ll manage complex patient needs, coordinate care, and help improve services in an outpatient setting while making a real impact in a culturally rich and vibrant city. Responsibilities Manage a caseload of patients with chronic conditions by assessing needs and creating individualized care plans. Coordinate care across providers, clinics, and community programs to ensure patients receive timely services and care. Provide education and coaching to patients and families to promote understanding, self‑management, and treatment adherence. Collect and maintain case‑management data, including service utilization, identified barriers, interventions, and patient outcomes. Use patient data to identify care gaps, improve coordination processes, and support quality‑improvement efforts. Requirements U.S. citizenship is required. Selective Service registration is required for males born after 12/31/1959. Individuals must not have been found guilty of or pleaded nolo contendere or guilty to certain crimes as per Public Law 101-630. Confidential financial disclosure form required. Background investigation and pre‑employment fingerprint check required; clearance must be obtained before hire. Successful pass of E‑Verify employment verification check. Set up direct deposit for payment. Background Investigation Selected candidates must undergo a pre‑employment fingerprint check and a comprehensive background investigation. Clearance must be obtained prior to the start of employment. Employment may be terminated if the investigation finds unsuitability or false statements are made. Qualifications To qualify, your resume must document sufficient experience or education to perform the duties of the position. Experience includes paid, unpaid, volunteer, or part‑time work. Basic Requirements A graduate or higher level degree, bachelor’s, associate degree, or diploma from an accredited professional nursing program is required. Official certification from the Commission on Graduates of Foreign Nursing Schools for those who graduated from foreign nursing schools. Current, active, full, and unrestricted license or registration as a professional nurse from an approved state or territory. Passage of the National Council Licensure Examination is required. For GS‑11 and above, additional experience may be required: either completion of a Ph.D. or equivalent doctoral degree, or at least one full year of professional nursing experience and possession of a diploma, associate degree, bachelor’s, or master’s degree from a professional nursing program. At least one year of specialized experience equivalent to the next lower grade level in the federal service, such as managing chronic patient caseloads, coordinating care, and using patient data for improvement. Additional Information Federal employees earn annual leave based on years of service. The position is covered by a bargaining unit and may include recruitment or relocation incentives.
