Pharmacist - San Fernando, CA

A 400-bed hospital in the San Fernando Valley region of Los Angeles is seeking a Pharmacist to support their Transitions of Care and Ambulatory Care services. This role is integral to ensuring patients receive the medication education and counseling they need before discharge, improving medication adherence and health outcomes.


Key Responsibilities:

• Provide medication education and counseling to patients prior to hospital discharge
• Assist in refilling medications as part of the 'meds to beds' program to facilitate seamless transitions of care
• Collaborate with healthcare teams to promote patient compliance with post-discharge medication regimens
• Support ambulatory care initiatives within the hospital network

Qualifications and Desired Experience:

• Licensed Pharmacist with relevant pharmacy credentials
• New PGY1 residents are preferred but candidates without PGY1 training will also be considered
• Experience or interest in Transitions of Care and Ambulatory Care pharmacy services is highly desirable
• Ability to work effectively within a large health system environment
• Commitment to patient education and medication safety
