Hospice of South Texas

Hospice of South Texas seeks an RN Case Manager to deliver comprehensive nursing care in the Victoria/Port Lavaca area. The role includes quick assessment, care planning, and patient-family education on disease process, nutrition, and end-of-life needs. The position requires two years of RN experience (hospice or home health preferred) and a valid RN license with a driver’s license.
