RN Case Manager - Home HealthLocation: Plainsboro, NJ and West Windsor, NJAssignment Duration: This is a long-term contract position.Schedule: 8:00 AM - 4:30 PMJob DescriptionWe are seeking an experienced RN Case Manager for our Home Health team. The role requires a dedicated professional with a strong background in case management and OASIS documentation. The successful candidate will manage a high volume of patients and documentation, ensuring quality care in the home health setting.ResponsibilitiesConduct comprehensive assessments and develop care plans for patients.Document patient progress and compliance with care plans.Coordinate care with interdisciplinary teams and facilitate transitions of care.Oversee 5-6 patient visits daily with a focus on quality and documentation.Provide wound care and treatment as needed.Required Experience / Certifications / LicensureMinimum of 2 years of home health experience.Experience in case management and OASIS is a must.Wound care experience is preferred.Current RN license in the state of New Jersey.Why ARMStaffing?At ARMStaffing, we take care of our employees!
