The Data Integrity Merge Analyst (HIM) is responsible for maintaining the accuracy, safety, and privacy of patient records within the electronic medical record (EMR/Epic) system. This role primarily focuses on investigating chart correction requests, resolving duplicate medical records, and conducting data audits to comply with regulatory standards and HIPAA guidelines. The analyst collaborates across departments to identify root causes of data errors, manages specific Epic workqueues, and processes specialized information requests like Care Everywhere workflows and physician case logs. Position Responsibilities Troubleshoots and triages chart corrections requests. Makes corrections or triage the error to appropriate analyst(s), end users. Monitors data integrity by reviewing and validating all potential patient matches. Verifies the information available in the patient records to ensure the integrity of the record. Identify and analyze duplicate medical records patterns, presenting leadership with data corrective workflows to improve front-end registration accuracy and present corrective actions to decrease the number of duplicate medical record numbers creation and maintain data accuracy. Monitors data integrity, merges duplicate medical records to ensure the integrity of the patient record and the status of duplicate creation and validate all potential patient matches. Is able to investigates and resolves potential duplicate workqueues in Epic to ensure every patient has an accurate single identifier patient records and safeguard data privacy and safety. Performs merges only on confirmed duplicate patient medical record #s timely and accurately following the established procedures. Able to determine root cause for data integrity issues and provides feedback on inappropriate patient creation and/or trends which cause duplicates within the EMR. Is able to update the patient status to deceased in the EMR system once notification of death and supporting documentation (death certificate) has been received. Ensure documentation is scanned into the EMR 100% of the time. Is proficient in managing Epic workqueues and executing duplicate reconiliation process. Is proficient in navigating the patient’s EMR and has the knowledge required for making corrections. Utilizes all chart correction tools available in the electronic medical record system as needed. Is accurate and confident in applying Known-Non-Duplicate (KND) status to valid unique patient profiles within established quality standards to ensure records are not incorrectly merged. Collaborates with IT, Patient Access, clinical teams, and other departments as needed to implement appropriate solutions for chart correction cases. Is able to conduct data audits and validations in Epic to ensure data meets quality standards and regulatory requirements (H&P, OP reports, Consents, and DC Summary). Consistently runs and analyzes the "MSMC Notes Not Shared with Patients" report in EPIC to identify, review and remediate improper documentation blocking and ensure patients receive full, timely access to their electronic health information. Accurately executes the "Care Everywhere" unlink workflow process and update patients Care Everywhere participation status in Epic within established turnaround times upon receiving Opt Out request from the patient. Responds to Care Everywhere ID requests within the established turnaround time upon receiving the request from the facility to ensure that the record is accessed and shared in compliance with HIPAA. Is able to accurately conduct chart reviews for Release of Information cases, such as insurance, audits, and birth defects requests, to ensure data integrity, accuracy, and completeness of the medical records being requested prior to release. Conducts case review and runs reports on physician case log requests. Cases may include physician credentialing/qualifications and/or board re-certification requirements. Responds to Case log email request within 5 business days and maintains "Physicians Case Log" request up to date. Is able to work with other healthcare professionals, such as doctors, nurses, and administrators, to ensure that patient records are accurate and complete. Correctly moves patient contacts and encounters through the "Contact Move" process to maintain data accuracy, protect patient safety and billing integrity. Able to review and validate approved encounters for selected cases through the appropriate EMR workqueue and prepare records for onsite review within the HIM Department. Excellent verbal and written communication skills. Able to communicate effectively with clerical, technical, and managerial level staff. Detail oriented, analytical and computer skills. Able to determine root cause for data integrity issues. Follows confidentially and HIPAA guidelines. Demonstrate knowledge and the ability to operate software applications such as Microsoft Office, Word, and Excel. Accepts and performs other duties as needed. Qualifications License/Registration/Certification None Education Associate’s degree or equivalent years of experience required. Experience 2 years of experience working with an EHR system with a strong focus on Health Information Management (HIM) required.
