Position Summary: This role plays a crucial role in ensuring continuity of care and addressing urgent patient needs during weekend business hours. The role involves being the initial point of contact for patients, families, or caregivers who require assistance or have concerns.

 

Schedule: Full-time weekend position working 30 hours per week on either Friday–Sunday or Saturday–Monday (three 10-hour shifts). Enjoy pay for 40 hours per week while receiving full-time benefits.

 

Education & Experience - Applicants must possess the following qualifications or an equivalent combination of education and experience:

• Education: New One of the following degrees required: ASN, BSN, MSN, DNP
• Experience: A minimum of six (6) months’ experience in hospital or homecare. Minimum of one-year experience in community health. University of Rochester Medicine Home Healthcare experience preferred. Knowledge of Hospice care a plus. Licensure, Certification, and/or Credential Requirements
• New York State Licensed RN

 

Essential Functions

Qualified individuals must be capable of performing the following essential duties and responsibilities, with or without reasonable accommodations:

• Provide phone triage for patient related calls using clinical knowledge and judgment. Ensure problem resolution at point of contact to the maximum extent possible and within provider ordered care. Execute good decision making and set priorities.
• Follows Agency mission, and administrative policies and procedures to ensure delivery of quality care.
• Work collaboratively with clinical teams to meet/exceed customer needs and maximize efficiencies.
• Utilize excellent customer care skills with internal and external customers, promoting positive, respectful communication to patients and families.
• Follows established care plans for patient to provide guidance within physician orders.

• Utilizes resources, supplies, and time efficiently to contain costs and increase the value of services to the community.

• Assures the completion of all appropriate clinical records needed for compliance with state and federal legislation and the delivery of seamless service within established time frames.
• Communicates timely to the health care team members the outcome of the triage call to ensure appropriate follow up occurs based on the needs of the patient/family’s plan of care.
• Maintains current knowledge of all available URMHC and community-based services.
• Ensures problem resolution at the point of contact.
• Responsible for the Key Performance Indicators (KPI’s) established by their manager.

• Participates in education including orientation, in-service training programs, and self-development, as needed.
• Demonstrates commitment, professional growth, and competency.

• Other duties as assigned. In the event triage calls are light, other duties may include:
  • Auditing
  • Transfers for in-patients
  • Process supply ordering

 

Salary range: $67,700.00- $115,045.00
