About the Role:

As a Licensed Practical Nurse (LPN), you will play a vital role in delivering high-quality care to our residents. Your expertise will be instrumental in creating a warm and welcoming environment, where our residents feel supported and cared for. You will work collaboratively with our interdisciplinary team to develop and implement individualized care plans, administer medications, and provide assistance with daily living activities.

Responsibilities:

• Provide direct patient care to residents, including but not limited to vital signs, medication administration, and wound care

• Develop and implement individualized care plans, taking into account each resident's unique needs and preferences

• Collaborate with the interdisciplinary team to ensure comprehensive care delivery

• Assist with daily living activities, such as bathing, dressing, and personal hygiene

• Monitor and report changes in resident condition to the supervising nurse and physician

• Participate in quality improvement initiatives and maintaining accurate records

Requirements:

• Current LPN licensure in the state of Pennsylvania

• High school diploma or equivalent required
