Job Description

Job Summary:

Provide specialized inpatient rehabilitation therapy to patients in a hospital setting, ensuring optimal recovery outcomes through personalized treatment plans and collaborative care.

Responsibilities:

• Assess patients' physical status and develop individualized treatment objectives.
• Administer therapeutic interventions such as evaluations, treatments, and mobility exercises.
• Document patient progress and clinical assessments accurately in the electronic medical record system.
• Collaborate with multidisciplinary teams to coordinate patient care and discharge planning.
• Adhere to hospital policies, safety standards, and dress code guidelines.
• Maintain certification requirements and participate in weekend rotations as needed.

Qualifications:

• Hold valid state licensure as a Physical Therapist.
• Possess hospital-based inpatient rehab experience.
• Demonstrate proficiency with EMR systems, preferably Meditech.
• Have excellent communication and teamwork skills.
• Be BLS certified and capable of working in an acute care environment.
• Experience with CARE scoring is a plus.
