In this role, you will:Conduct thorough evaluations of patients in either inpatient or outpatient environments and develop tailored treatment plans.Implement physical therapy interventions focused on improving mobility, strength, balance, and function.Collaborate proactively with nurses, physicians, case managers, and other rehabilitation professionals to enhance patient outcomes.Educate patients and their families about therapeutic objectives, discharge planning, and home exercises.Maintain detailed documentation of evaluations, treatments, and progress as per regulatory and facility guidelines.Engage in care team rounds and interdisciplinary conferences to promote coordinated care.In terms of education, we're looking for candidates with a Doctorate in Physical Therapy (DPT) or a closely related accredited degree. State licensure as a Physical Therapist is required, and while a CPR certification is preferred, any specialized training in areas such as neurology or geriatrics is a bonus.
