Registered Nurse (RN) - Home Health

Registered Nurse – Home Health

Schedule: 8hrs

Assignment Details:

  • Contract Length: 14 weeks
  • Guaranteed Hours: 40

Requirements:

  • Current state license as a Registered Nurse
  • Homecare Homebase Software Experience
  • OASIS Experience
  • 1-2 years experience as a Registered Nurse in a clinical care setting
  • 2+ years Home health experience
  • Proof of current CPR certification
  • Current driver's license and reliable transportation with insurance

Roles & Responsibilities:

  • Assess and respond to the needs of home health and/or hospice patients in stable and emergent situations.
  • Implement, develop, document, and evaluate the Plan of Care (POC) to ensure quality and continuity of care.
  • Educate patients and/or their families regarding care, medication, and safety measures.
  • Supervise and provide clinical direction to support staff to ensure effective service delivery.
  • Monitor assigned cases for compliance with third-party payer requirements.
  • Provide effective communication as a liaison to patients, families, and healthcare team members.
