Registered Nurse - Hospice

Travel Registered NurseSeeking an experienced Travel Registered Nurse to deliver compassionate, high quality end of life care to patients in the home setting. This role focuses on patient assessment, care coordination, and collaboration with interdisciplinary teams to ensure comfort, dignity, and quality of care for hospice patients.Schedule & Work Details:Location: Coverage across 6 counties, with attempts to assign a consistent geographic area based on census and daily needsSchedule: Monday through Friday, 8:00 AM to 4:30 PMWeekly Hours: 40 hoursOn Call/Weekends: Required, rotating with teamPatient Load: Approximately 7 to 8 patients per day, may varyPay Rate: $51 to $53 per hourMileage Reimbursement: Yes, reimbursed between visits (not including commute to first visit or return home)Dress Code: Preferred gray or black scrubs, non bright colorsKey Responsibilities:Assess and manage hospice patient care in the home setting in accordance with care plans and physician directionPerform medication reconciliation and coordinate medication refills as neededComplete weekly Interdisciplinary Group (IDG) documentation and participate in IDG meetingsCommunicate effectively with physicians regarding patient condition, changes, and care needsProvide patient and family education on disease progression, symptom management, and care expectationsMaintain accurate and timely documentation using Homecare HomebaseCollaborate with interdisciplinary team members to ensure coordinated and compassionate careRespond to on call needs and provide support outside of scheduled hours as requiredRequired Qualifications:Active, unencumbered RN license in Tennessee or compact stateRecent hospice nursing experience requiredExperience with Homecare Homebase (HCHB) documentation systemAbility to work independently as well as collaboratively with care teams and physiciansStrong clinical judgment and decision making skillsAbility to manage time effectively and prioritize patient needsValid driver's license and reliable transportationPreferred Qualifications:Strong organizational and communication skillsAbility to adapt quickly in a dynamic travel assignment settingExperience balancing productivity while maintaining high quality patient carePride-Health offers eligible employee's comprehensive healthcare coverage (medical, dental, and vision plans), supplemental coverage (accident insurance, critical illness insurance and hospital indemnity), 401(k)-retirement savings, life & disability insurance, an employee assistance program, legal support, pet insurance and employee discounts with preferred vendors.
