Case Manager, Park Slope Women's Shelter

Overview CAMBA is a community organization offering more than 180 integrated programs across Education & Youth Development, Family Support, Job Training & Employment Support Services, Health, Housing, and Legal Services. The organization serves nearly 80,000 individuals and families, including about 13,000 youth, and works with a diverse cross section of New Yorkers. More than half of CAMBA's clients are immigrants and refugees, with over 85% of families living in poverty. CAMBA operates six single adult shelters, six family shelters, and one drop-in center. Each shelter provides a supportive, structured, therapeutic, safe, and drug-free environment to help clients stabilize and transition to permanent or supported housing. Position Case Manager Location: Brooklyn, NY Reports To: Supervisor II (Clinical) Responsibilities What the Case Manager Does: Under general supervision, the person in this position is expected to: Immediately formulate a plan and prepare clients for permanent housing placement. Engage individuals requiring social service assistance. Interview and evaluate clients, formulate Independent Living Plans and goals. Locate and refer to appropriate community resources for clients. Maintain professional relationships with clients and client confidentiality. Practice Universal Precautions/Standard Protocol & Procedures. Comply with Federal, State, City, and CAMBA security and privacy policies protecting health information. Conduct initial intake and assessment of clients’ needs and periodic re-assessments. Create and maintain client electronic and paper files. Prepare initial psychosocial evaluation and update according to regulations. Obtain and maintain documentation from clients (e.g., birth certificates, proof of income, medical documents, release forms). In collaboration with clients, revise independent living plans with short- and long-term goals. Assist clients in attaining goals by identifying community resources and referring to services inside and outside CAMBA (e.g., on-site medical and psychiatric services). Help clients overcome barriers to goal attainment. Provide support, training, and coordination with other social services staff. Advocate for clients to gain housing stability; encourage active participation in the process. Monitor progress toward goals through regular face-to-face contacts/sessions. Document all client encounters using the DHS CARES Database and paper files. Ensure CARES Database reflects client service requests (e.g., car fare, appointments, referrals). Follow up with clients and referral organizations regarding progress. Provide information for weekly, monthly, quarterly, and annual reports. May act as client liaison/advocate with outside organizations on education, healthcare, housing, and legal issues. May schedule and escort clients to appointments with referral organizations. May assist clients in completing applications for benefits and entitlements. May follow up with clients after completion of primary goals to ensure stability. May prepare marketing materials for the program. Tasks may be modified, expanded, or reassigned over time. Qualifications Education, Licenses and/or Certifications Required: Bachelor's degree (e.g., B.A., B.S.W.) and/or equivalent experience. Minimum Experiences and Skills Required: May be required to become First Aid/CPR certified. May be required to become certified in overdose prevention. Bi-lingual preferred. Preferred Qualifications: Bi-lingual English and Spanish (example). Pre and/or Post Employment Requirements: TB test required. Compensation & Status Compensation: $44,800-$48,925 annually When salary ranges are listed, the range represents the low and high end for the position and program. The salary offered is based on factors including experience, education, budget, program size, internal equity, skills, and other requirements of the organization.
