Position Overview The LPN is responsible for providing quality patient care and performing skilled care in the patient’s home. Responsibilities Provide services within the scope of practice as defined by state nursing laws, in accordance with the plan of care, and in coordination with the healthcare team. Instruct the patient’s family in patient care and maintain a healthy environment. Observe, record, and report to the supervising nurse and/or physician the patient’s response to treatment and changes in the patient’s condition daily. Collaborate closely with physicians and RNs to document care accurately and promptly; may not alter the plan of care. Qualifications One year of clinical LPN experience. Current LPN license in the state where services are rendered. Current CPR certification. Driver’s license, vehicle insurance, and access to a dependable vehicle or public transportation. Ability to reliably commute to multiple patient residences daily within the assigned geographic area.
