LPN LTCTryfacta is seeking a LPN LTC for our client in Palmyra, MO 63461. This is a 13 week contract assignment. If you meet the qualifications listed below and are interested, please apply now!Position Title: LPN LTCLocation: Palmyra, MO 63461Duration: 13 weeks (possible extension)Work Schedule: 8 hours evening with every other weekend.Responsibilities for this position include, but are not limited to:Provide direct nursing care to residents in a long-term care setting.Administer prescribed medications, treatments, and injections safely and accurately.Monitor, assess, and document residents' health conditions and report changes to the RN or physician.Assist with developing and implementing individualized care plans.Perform wound care, dressing changes, catheter care, and other nursing procedures within LPN scope of practice.Maintain accurate and timely documentation in the electronic medical record (EMR).Collaborate with physicians, RNs, CNAs, therapists, and other healthcare professionals to ensure quality patient care.Supervise and delegate appropriate tasks to Certified Nursing Assistants (CNAs).Respond promptly to changes in resident conditions and medical emergencies.Educate residents and their families on treatments, medications, and ongoing care needs.Follow all facility policies, infection control guidelines, and safety regulations.Maintain a clean, safe, and comfortable environment for residents while promoting dignity and independence.To be considered for this position, you should have:Active LPN LicenseLTC/SNF/Nursing Home experience preferredMedication administration experienceEMR documentation skillsBLS/CPR certificationAbility to work evening shifts and every other weekendStrong patient care and communication skillsTryfacta is an Equal Opportunity/Affirmative Action Employer.
