Green Hills Center for Rehabilitation and Healing Social Worker Join our team as a Social Worker! Green Hills Center for Rehabilitation and Healing is seeking a compassionate and dedicated Social Worker to join our interdisciplinary team. In this role, you will play a vital part in supporting residents and their families by addressing psychosocial needs, facilitating care planning, and enhancing overall quality of life within our skilled nursing environment. Key Responsibilities Provide emotional support and counseling to residents and their families Conduct psychosocial assessments and develop individualized care plans Coordinate discharge planning to ensure smooth transitions of care Advocate for residents’ rights and overall well-being Collaborate with nursing, therapy, and medical teams to support holistic care Assist residents and families with community resources and services Maintain accurate and timely documentation in accordance with regulatory standards Participate in care plan meetings and interdisciplinary team discussions Qualifications Bachelor’s Degree in Social Work (BSW) required; Master’s Degree (MSW) preferred Prior experience in a skilled nursing facility or healthcare setting is a plus Strong understanding of discharge planning and care coordination Excellent communication and interpersonal skills Ability to handle sensitive situations with professionalism and empathy Knowledge of state and federal regulations preferred What We Offer Competitive salary (based on experience) Comprehensive benefits package (medical, dental, vision) Paid time off and holiday payOpportunities for professional development and advancement Supportive and team-oriented work environment Apply Today!
