Our RN Case Managers play a central role in ensuring our clients receive exceptional care and our staff feel supported.Position SummaryThe RN Case Manager is responsible for overseeing a caseload of home‑health patients, completing skilled nursing visits, developing individualized care plans, coordinating interdisciplinary services, and ensuring compliance with state and federal regulations. This role is ideal for a nurse who thrives in a flexible, autonomous environment and enjoys building meaningful relationships with clients and families. Key Responsibilities Complete initial and ongoing in‑home assessments, identifying physical, psychosocial, and environmental needs.Develop, implement, and update individualized plans of care in collaboration with physicians and the care team.Provide skilled nursing services, including wound care, medication management, disease‑specific interventions, and patient education.Communicate changes in condition to physicians and obtain updated orders as needed.Document all care accurately and timely in the EMR, including OASIS assessments when applicable.Participate in interdisciplinary case conferences to ensure coordinated, high‑quality care.Educate patients and caregivers on disease processes, safety, medications, and self‑management strategies.Support safe discharge planning and continuity of care.Qualifications: Active Registered Nurse (RN) license in the state of IllinoisMinimum 1 year of nursing experience; home‑health experience preferred.Strong clinical judgment and critical‑thinking skills.Excellent communication, organization, and documentation abilities.Valid driver's license and reliable transportation.
