Job Summary: The Behavioral Health Specialist is a Qualified Mental Health Professional (QMHP) who provides services, primarily in the community and clinic-based settings, as part of a multidisciplinary behavioral health team to coordinate care for adults. This position will be located in the community and at Black Men's Health Clinic. The Behavioral Health Specialist participates in diagnostic evaluation, completes assessment, and collaborates with individuals to define and facilitate desired life outcomes utilizing a recovery based and trauma-informed perspective. The Behavioral Health Specialist delivers therapeutic interventions and provides individual psychiatric rehabilitative skills training and crisis assessment/management in the consumer's home, clinic, or in the community environment. This service is provided through ongoing assessment, preliminary diagnosis, and ongoing treatment of a consumer's presenting Axis I psychiatric and co-occurring disorder under the supervision of the treating psychiatrist/APN and the Program Manager. The position provides coordination of community resources to assist consumers in accessing medical, social, educational, financial and other appropriate services that help the individual achieve an improved quality of life, while decreasing the need for inpatient hospitalization and arrests. At this advanced level, consumer caseloads are complex and dynamic with increased need for crisis intervention, and the Behavioral Health Specialist helps individuals develop skills to manage symptoms, obtain/maintain employment and housing, and/or to obtain services such as medical care, nutritional assistance, financial assistance, transportation, legal assistance, and other resources to fulfill needs.

Qualified Mental Health Professionals (QMHP) embrace trauma-sensitive and informed practices, provide Suicide Safe care, and foster a supportive and welcoming environment. QMHPs operate in a dynamic environment, develop extensive knowledge of community resources, collaborate with interdisciplinary teams, and cultivate a clinical skill set by forging therapeutic relationships with community members and/or stakeholders. Interventions and services can be both office- and community-based and may require the use of a personal automobile. Individuals served may be diagnosed with mental illness, co-occurring substance use disorders, chronic health conditions, and/or IDD. Staff members in this position are working under the supervision of a clinically licensed professional who provides ongoing evaluation and oversight of all assessments, diagnoses and treatment. At this level, clinical assignments may be of moderate complexity and are handled with considerable independence, and complicated cases receive closer clinical review and supervision. Services provided may include individual, family and/or group skills training using evidence-based interventions, crisis assessment, and case management. Work is performed under general supervision with moderate latitude for the use of initiative and independent judgment.

Essential Duties & Job Responsibilities:

• Responds to client-related concerns.
• Evaluates the strengths, preferences, functional impairments, suicide risk, and symptoms of psychiatric and/or co-occurring disorder(s) of assigned individuals through formal assessments, documentation in electronic medical records (EMRs), face-to-face interviews, and client self-report. • Attend and actively participate in team meetings, informing the supervisor if unable to attend.
• Demonstrates flexibility and teamwork to promote a healthy work environment.
• Collaborates with interdisciplinary team: Collaborates with assigned individuals to develop, review, and/or revise care plans consistent with training standards in a timely manner.
• Tailors the plan according to agency and accrediting and funding agencies' policies/procedures for the client being served (Reference Peer and Manager Quarterly Audit Tool for full list of requirements)
• Proficiency in therapeutic interventions. Maintains working knowledge of both Integral Care and external services and resources that enhance the recovery of individuals served.
• Uses collaborative documentation to complete timely and accurate documentation of all services and contracts in accordance with agency policies and procedures (OP 12.05: Guidelines for Making Electronic Health Record Entries)
• Initiates and maintains professional communication with each client's interdisciplinary treatment team, external service provider(s), and natural supports to ensure holistic care.
• Engages in face-to-face service provision with assigned clients at the frequency required by the designated program's policy.
• Provide optimal trauma-informed care by completing all required training and consistently incorporating trauma-informed principles into service delivery as applicable, including, where appropriate, recognizing the signs and impacts of life-changing experiences, demonstrating awareness and sensitivity in interactions, creating a safe and supportive environment, and applying best and promising practices in work with clients, families, staff, and others.
• Provide suicide-safer care by completing all required training; responding appropriately to and managing crisis situations, including assessing risk of harm to self or others and implementing necessary safety measures; supporting access to suicide prevention and intervention services for all individuals; and, for clinical roles, completing required screenings, safety planning, and caring contacts in accordance with policy and practice standards.
• Provides optimal service to all clients, both internal and external.
• Responds promptly, professionally, and respectfully to client needs, requests, and concerns; communicates clearly; maintains confidentiality as appropriate; and collaborates effectively to resolve issues and support the health, dignity, independence, and success of those served.
• Provide bilingual services, when applicable, by communicating directly with individuals of limited English proficiency in the target language or by facilitating access to appropriate language assistance resources, in a professional and effective manner

Regular, predictable attendance is considered to be an essential requirement for this position. Other duties may be assigned. May require a flexible work schedule.

Education:

Required:

• Graduated from an accredited college or university with a Bachelor's degree with a major specialization in Human/Behavioral/Social Services or related field.

License:

• Current and valid driver's license and a clean driving record.

Hours: Full-Time.

Workdays: Monday - Friday.

Workhours: 8:00AM - 5:00PM.

Salary: $24.18 - $25.39 an hour based on education.
