About the Role
As a Home Health Physical Therapist , you’ll deliver one‑on‑one, high‑touch care to patients in the comfort of their homes. You’ll have the autonomy to manage your schedule, build meaningful patient relationships, and make a direct impact on outcomes.
Responsibilities
- Skilled therapy visits — perform assessments and deliver individualized interventions
- Care plan management — create, update, and oversee patient‑specific treatment plans
- Interdisciplinary collaboration — work closely with families, nurses, and other providers
- Support patients in regaining mobility, independence, and quality of life
✅ Requirements
- 2+ years of PT experience
- 2+ years of Home Health experience (required)
- Active Physical Therapy license
- Willingness to obtain Florida licensure
