NLCS is a highly rated skilled nursing operation providing cutting-edge and high-quality health care services for elders in the Rockville community.What will I do as the LPN MDS Coordinator?As an LPN MDS Coordinator, you'll be at the heart of their mission-ensuring accurate assessments, supporting care planning, and helping our residents achieve their highest quality of life. Every day is an opportunity to make a difference, collaborate with a dedicated team, and advance your career in a supportive, resident-centered environment.Demonstrate commitment to NLCS' core values in all aspects of your workSupport NLCS policies, procedures, guidelines, and code of conduct standardsCultivate an inclusive and supportive environment that empowers team members and residents to thrive and pursue personal and professional growth, in alignment with our philosophy of purposeful livingFoster a positive work environment that promotes team member engagement and satisfactionCoordinate individualized plans of care using an interdisciplinary team approachReview and evaluate resident MDS assessments and care plans to ensure proper implementationParticipate in resident care plan conferencesMaintain the MDS schedule within required time frames and communicate pending assessments to relevant directorsGenerate and distribute the monthly MDS calendarComplete and audit MDS assessments, ensuring all required signatures are obtainedTransmit electronic copies of assessments to the Centers for Medicare and Medicaid Services (CMS) and reconcile all transmissionsQualifications:Current LPN license in the State of MarylandMDS certification or proven experience completing MDS assessmentsMinimum 2 years of clinical experience.
