Why does the job exist?

This position will provide Nursing services to individuals who are developmentally disabled. The LPN nurse will evaluate with the guidance of a RN, the physical, mental, and emotional needs of individuals; implement health care plans and crisis plans as appropriate; complete documentation; administer medications; coordinate medical services with physicians and members of the treatment team; and address any changes in the medical condition of the individual. This position will also provide on-going training to treatment staff, as well as, schedule, individuals appointments. Position will be required to work shifts, weekends, holidays, overtime and call back hours as needed.


How does it get done?

This position is completed by working hands on with the individuals served, direct care staff, and the internal and external teams. This position focuses on the health and well-being of individuals with developmental disabilities. This includes but is not limited to medication pass, assisting RNs with evaluations, daily documentation, training's, wound care, and taking direction from the RN.


Who are the customers?

Adult individual's who are developmentally disabled and could display behavioral challenges and/or medical challenges.


Ideal Candidate

Experience working with developmentally disabled individuals
Experience developing and monitoring patient care plans
Experience with electronic charting
Experience working with multidisciplinary teams


Minimum Qualification

Licensure as LPN or LVN.


Employment Requirements

This position is designated as safety sensitive. Employment is subject to pre-employment and random testing, in accordance with all terms and conditions of federal and state law, rules and regulations in relation to alcohol and/or drug testing. Employment is subject to a criminal background check and is conditional pending results. Must possess and maintain a valid New Mexico Driver's License. Must possess and maintain a current Defensive Driving Course Certificate from the State of New Mexico or must pass and receive a Defensive Driving Course Certification as a condition of continued employment. Successful completion of Health Insurance Portability and Accountability Act (HIPAA) training is required.


Working Conditions

Work is performed in the homes of community based consumers with noise levels that can range from moderate to loud. Position requires the ability to lift/push/pull/transfer and/or carry up to 50 pounds without assistance and up to 100 pounds with assistance. Local daily driving for outreach and travel to attend training sessions and meetings. "Specific geographic location for this position will vary depending on agency needs and utilization."
