Overview The role of the Transitional Care Nurse involves coordinating patient transitions from hospitals and facilities to home care, ensuring seamless communication across settings. Responsibilities Support patient self-management, advocacy, education, coaching, and counseling for patients and families. Utilize the nursing process for ongoing assessment and outcome management. Collaborate with the care team for patient-centered planning. Qualifications BSN and active RN licenses in MO and KS with at least 2 years of experience. Experience in Home Health, Hospice, or Case Management is preferred. An active BLS certification and a clean driving record are necessary. Schedule & Coverage The role covers the Kansas City Metro and rural areas, with flexible scheduling and occasional telemonitoring.
