As a Home Health Licensed Master Social Worker, you'll provide compassionate guidance, advocacy, and emotional support while helping patients overcome barriers that affect their health, safety, and quality of life. Your work helps patients remain independent, strengthens families, and ensures no one has to face life's challenges alone. What You'll Do Complete comprehensive psychosocial assessments Identify emotional, social, environmental, and financial barriers to care Connect patients and families with community resources and support services Assist with discharge planning and long-term care planning Provide counseling, education, crisis intervention, and emotional support Collaborate with physicians and interdisciplinary team members Advocate for patients while coordinating appropriate community services Complete timely and accurate clinical documentation Qualifications Active Virginia Licensed Master Social Worker (LMSW) License Required Master's Degree in Social Work from an Accredited Program Valid Driver's License and Reliable Transportation Strong Assessment, Communication, and Documentation Skills Home Health or Healthcare Experience Preferred but Not Required Compassion for Serving Diverse Patient Populations Why Human Touch Home Health?
