Senior Home Health Nurse Case Manager

PACE Southeast Michigan

PACE Southeast Michigan is seeking a Registered Nurse Case Manager (RNCM) to utilize a systematic approach in delivering comprehensive care for frail elders. The RNCM will assess needs, develop care plans, and lead nursing staff while promoting autonomy and functional capacity. This role requires effective collaboration with participants and a multidisciplinary team. Responsibilities include medication administration, monitoring health status, and advocating for participants’ rights. A valid Michigan nursing license and experience with the elderly are essential for this position. #J-18808-Ljbffr
