As a Physical Therapist, you will play a critical role in evaluating and rehabilitating patients with physical functioning disorders and/or wound care. You will develop and implement personalized care plans following all regulatory and clinical practice standards. Your responsibilities will include:

Screening and evaluating patients to develop appropriate rehabilitation plans
Administering procedures such as manual techniques, ambulation, therapeutic exercises, and wound care
Adjusting treatments based on patient progress and documenting outcomes
Providing consultation and counseling to patients, families, and caregivers
Communicating effectively with the healthcare team regarding patient progress
Documenting all required information in the electronic documentation system
Participating in patient care conferences and required meetings
Supervising and supporting physical therapist assistants and rehabilitation aides
Adhering to APTA and State Required practice acts and ethics

Requirements:

Need 1 year experience in setting
LTC or Rehab experience required
Oklahoma State License required
New Grads OKAY with Level II experience
