Position Overview
Provide primary and acute care services in an employer sponsored primary care and wellness clinic. The provider will focus on proactive care, early intervention, chronic disease management, and long term wellness planning while maintaining strong confidentiality standards.

Key Responsibilities

• Provide primary and acute care services to patients ages 2 and older
• Prescribe medications as clinically appropriate
• Manage chronic conditions and support long term wellness goals
• Provide early intervention services and ongoing patient education
• Collaborate with clinical teammates to support a positive clinic culture

Required Qualifications

• Candidates must have at least two years of clinical experience within the past five years to qualify
• Active, unrestricted Nurse Practitioner or Physician Assistant license in Utah
• National board certification as an FNP or PA
• Active DEA registration
• Experience treating patients ages 2 and older
• Ability to pass onsite credentialing

Preferred Experience

• Primary care experience
• eClinicalWorks (eCW) experience preferred
