Qualifications:

Must be licensed (in good standing) as an LPN in the State of Illinois.

Must possess a current CPR Certification.

Prefer 2-3 years nursing experience

Essential Responsibilities:

Administer all medications, treatments, blood sugar monitoring and other medical care to residents as prescribed by the physician/psychiatrist, according to the facility’s specific policies and procedures.

Provide emergency care services, as needed.

Document in the patient’s medical record.

Provide emotional support and counseling to all residents, as needed.

Provide resident education regarding medications, treatments and other medical care, as needed.

Notify physicians of abnormal resident condition, lab values, vital signs, behavior, incidents, and/or reactions to medications.

Ensure that vital signs, such as blood pressure, temperature, and weight are taken

Care Plan experience preferred

1- 10a-10:30p and 1- 10p-10:30a including every other weekend
