Occupational Therapist

Job Title: Occupational Therapist

Duration: 13 weeks

Shift: Day Hours per Shift: 8:00 AM - 4:30 PM

Experience: Minimum of two years of experience; minimum of six months in home health

License: Licensed Occupational Therapist

Certifications: Valid Driver's License; Proof of Automobile Insurance; Basic Life Support (BLS)

Must-Have: Patient assessment, Clinical documentation, Supervision, Knowledge of home health regulations, Ability to operate ultrasound machine and other therapy equipment

Description: The Occupational Therapist is a member of the patient care team within the Department of Home Care and is directly accountable to the Patient Clinical Manager. The position requires that the individual be clinically competent and knowledgeable of home health regulations. Demonstrates knowledge and skills to appropriately communicate and interact with patients and families while being sensitive to their cultural and religious beliefs. The therapist assists in evaluating the level of function and helps develop the plan of care and make revisions as applicable. Prepares clinical and progress notes and advises and consults with the patient, family, and other members of the health care team to develop individualized measurable, realistic short-term and long-term goals for patients. Supervision of the COTA according to regulations. Performs specific therapy procedures in the home according to the plan of care. Knowledgeable and compliant with the 30-day patient reassessment requirement. Participates in in-service education and continuing education as applicable. Prepares documentation of patient care in the home at the time of visit as applicable and submits documentation promptly according to policy. Notifies the physician of any changes and instructs the patient and family in meeting needs. Performs other job duties as assigned.

Performance Expectations: Demonstrates ability to operate an ultrasound machine by selecting appropriate settings, inspecting and troubleshooting any problems with equipment, and performing safe treatments. Demonstrates ability to enter all patient charting in electronic medical records documentation timely while meeting state and federal regulations. Must have the ability to assess, plan, and coordinate patient care, work independently, carry out orders accurately, demonstrate initiative, prioritize, have good communication skills, and maintain rapport. Must be able to operate various equipment and machines, which include E-stim, TENS unit, Hoyer lift, wheelchair, hospital bed, and others. Must be able to lift, handle, push, and pull in excess of 50 pounds.
