As a Nurse Practitioner or Physician Assistant with a background in Geriatrics or Internal Medicine, you'll play a vital role in delivering high-quality preventative care to residents in skilled nursing facilities. This hybrid role combines on-site visits with remote work from home, supported by a dedicated scribe team for efficient documentation.

Responsibilities:

• Build strong relationships with facility staff and maintain a patient-centered approach
• Provide high-quality preventative care to residents in skilled nursing facilities
• Collaborate with the care team to develop individualized care plans
• Conduct on-site visits and remote work from home, supported by a dedicated scribe team
• Maintain accurate and timely documentation of patient visits and care plans

About the Role:

• This part-time opportunity offers 7 hours of work per week, with a flexible schedule and collaborative work environment

Requirements:

• Active NP or PA license (in the state of practice)
• 1+ year of experience in Geriatric care and Documentation review
