Develop individualized treatment plans based on patient needs, diagnoses, functional goals, and rehabilitation progress. Provide therapy to improve communication, cognition, voice, and swallowing safety, including patient and caregiver education. Document assessments, treatment outcomes, progress, and discharge recommendations while collaborating with the interdisciplinary care team.
