Essential Duties:
The TeleCare Team Respiratory Therapist is responsible for patients with respiratory and respiratory–related equipment and for assessment of patient / client need and the selection, instruction, and troubleshooting of appropriate equipment and / or service provided to meet those needs and maintaining appropriate documentation
Responsibilities:
- Receives and documents the order for respiratory equipment and / or Chronic Care Programs from the customer service, patient / client, caregiver, physician and family member.
- Responsible driving from office to patient home or hospital.
- Enroll patients into the appropriate Chronic Care Program as well as assess and educate patients on disease process.
- Ensures comprehensive documentation of each patient interaction and follow-up in the SCH charting platform while actively coordinating with the team on escalated cases.
- Collects additional information and / or authorization if referral is from other than the physician
- Involves the patient / client and / or family, caregiver in the process.
