Position Title: Director of Wellness

Supervisor’s Title: Administrator

General Statement of Responsibilities: Plans, develops, organizes implements, evaluates, directs the day-to-day function of the nursing department in accordance and compliance with the established policies, procedures, and practices, and in compliance with Federal, State, Local, and licensing regulations. Assured quality assurance and quality of resident’s care is provided in the nursing department. Ensures adequate staffing patterns and staff are qualified and trained. Responsible for documentation for treatment and nursing care.

Bona Fide Occupational Requirement: Must possess a nursing degree from a credited college or university or diploma in nursing from an accredited LPN program. Must possess a current unencumbered license to practice as a registered nurse in the Commonwealth of Pennsylvania.

Major Job Duties:

1. Responsible for developing and maintaining nursing service objectives, standards of nursing practice, in accordance with nursing policies and procedures.

2. Assess each residents needs and assist in the development of a treatment plan for nursing care to meet the daily treatment and care needs of the residents.

3. Participate in the interdisciplinary team, work cooperatively with team members to ensure coordination with the care and treatment for the residents.

4. Recruit, hire, and train staff in the nursing department. Explain personnel policies and individual duties to each new employee and assist with orientation of the facility and their job.

5. Ensure that nursing personnel provide treatments, medications, diets, rehabilitative nursing care, activities of daily living, protective service, and training in self-care as stated in the care plan to a certain that optimal comfort and care is provided.

6. Supervise and evaluate the performance of the staff in the nursing department. Supervision of staff includes monitoring performance, ongoing training, handling and resolving behavior, and performance problems, disciplining and terminating staff as necessary.

7. Ascertain that physicians accompanied by licensed personnel are visiting on a regular basis as required and notify the ones that are not attending.

8. Ensure the nursing department is adequately staffed and prepare staff schedules in the nursing department.

9. Organize and implement a system for documenting nursing services provided to residents in compliance with regulations and standards, and incompliance with.

10. Make sure personal rounds of entire resident load daily are to be acquainted with the resident’s problem (s) with both physically and emotionally and to make sure staff is providing appropriate care.

11. Schedule conferences with residents and families when necessary to interpret policy and smooth relationships pertaining to nursing services.

12. Refer residents and families to social workers and/or proper community agencies when necessary.

13. Plan, organize, direct, monitor the infection control program for the facility; Any ensure any incidents of noncompliance are immediately corrected.

14. Develop procedures and practice for emergency incidents; and enact emergency procedures.

15. Due chart audits of a sample number of charts monthly to a certain proper charting and documentation of caregiver and directly confer with those employees who are not properly documenting.

16. Encourage continuous staff development of staff in the nursing department; provide Inservice training as needed when possible.

17. Assist and maintain quality assurance data and preparation of reports for Quality Assurance Committee.

18. Participate in meetings, Inservice training, and staff development events.

19.Develop, prepare, and submit report of nursing services as requested and required.

20. Establish and maintain positive, effective working relationships with other Department Heads, outside agencies, and the medical community.

21. Maintain relevant, up to date manuals and reference materials for use in the nursing department.
