RN Field Care Manager Nurse

About This Role The RN Field Care Manager Nurse assesses and evaluates members' needs to achieve and maintain optimal wellness by guiding members and families toward appropriate resources. This role is ideal for experienced nurses who thrive in a dynamic environment and are skilled in case management. Highlights Pay : $71,100 - $97,800/year Location : Detroit, MI Residency : Must reside in Michigan (Wayne or Macomb Counties) Work Style : Field-based Travel : 75-90% travel within Wayne and/or Macomb Counties What You'll Do Manage a caseload and complete assessments with members in their home or community-based settings. Provide clinical support and guidance for members with medical complexity. Develop and coordinate care plans to ensure members receive appropriate services. Address barriers to care and advocate for optimal member outcomes. Review, assess, and complete medical complexity attestations and clinical oversight activities. Ensure members receive services in the least restrictive setting. Develop and modify the Individual Care Plan involving applicable members of the care team. Support members and/or caregivers using an interdisciplinary approach to access services. Serve as the primary point of contact for the Interdisciplinary Care Team (ICT). Coordinate with members, ICT participants, and external resources to meet member needs. Requirements Education: Active Michigan license as a Registered Nurse (RN) or Advanced Practice Registered Nurse (APRN), including Nurse Practitioner (NP) or Clinical Nurse Specialist (CNS). Experience: 2+ years' experience in Health Care and/or Case Management. Experience working with the adult population and disease management.
