Ongoing duties combine remote work, office time, and direct field-based community engagement across Santa Barbara and San Luis Obispo Counties.JOB SUMMARY:The Bilingual Care Manager is responsible for providing care coordination for Medi-Cal members with complex medical, behavioral, developmental, and social needs. Serving the Central Coast region, this position acts as the primary liaison between families, pediatric providers, behavioral health teams, schools, and community organizations. The Care Manager supports outreach, intake, enrollment, and ongoing service navigation, ensuring trauma-informed and culturally responsive care. The ideal candidate demonstrates strong communication, organizational, and interpersonal skills with a deep commitment to community advocacy. This role features a combination of remote flexibility and direct field-based community engagement. The position is also required to adhere to CalAIM, CenCal Health, DHCS, HIPAA, and other regulatory agency guidelines as they relate to care management duties. Employees are responsible for following all company policies and procedures, including adherence to all safety and compliance standards in performing the functions below. ESSENTIAL FUNCTIONS:Client & Family Relationship Management: Conduct outreach, intake, and enrollment with eligible members & families in English and Spanish. Build and maintain strong, trusted, and professional relationships with members. (Example: children, parents, and guardians.) Conduct required face-to-face field visits (including home visits, community contacts, and accompanying or meeting members at medical appointments) at least once per month per member, or as mandated by CenCal Health and CalAIM guidelines. Complete comprehensive child, family, and member assessments to identify complex medical, behavioral, and social risk factors. Develop, implement, and monitor individualized, member-centered ECM Care Plans. Identify member or family needs, resolve service gaps promptly, and escalate complex clinical or social matters to leadership when needed.Operational Support & Care Coordination: Coordinate care across pediatric providers, medical specialists, behavioral health systems, schools, and social programs. Assist members and families with appointment scheduling, referrals, and navigation of community resources. Connect members and families to vital community supports, addressing challenges like housing instability and food insecurity. Participate in interdisciplinary care team meetings and case conferences to ensure seamless care integration. Maintain manageable caseloads to ensure high-quality, impactful service delivery. Technology & Documentation: Accurately document all care management activities, contact notes, and milestones within the designated tracking systems. Ensure full compliance with CalAIM, CenCal Health, and DHCS reporting and documentation standards. Utilize remote communication tools, telehealth platforms, and electronic care systems effectively.Other Duties: Represent Safelife at community resource events and collaborative agency meetings. Support internal process improvements to enhance the family experience and care efficiency. Perform other related duties as assigned.PHYSICAL/MENTAL DEMANDS, ENVIRONMENT: Must have excellent interpersonal and customer service skills. Must be comfortable working independently in the field and directly within the community. This position operates as a hybrid model, combining field-based home/community visits, office time, and remote work. Must maintain a valid drivers license, reliable transportation, and auto insurance for travel within Santa Barbara and San Luis Obispo Counties. EQUIPMENT USED: Computer / Laptop / Tablet Smart Phone / Hotspot Care Management Portals and Documentation Platforms SUPERVISORY RESPONSIBILITIES:None MINIMUM QUALIFICATIONS: Language: Fluent in English and Spanish (written and verbal) is required. (Mixteco fluency is a major plus). Location: Must currently reside in Central California (Santa Barbara or San Luis Obispo County). Experience: Minimum of 2 years of experience in case management, care coordination, community health, social services, or child/family services. Real-world lived experience supporting the community is highly valueda formal degree is not required. Skills: Experience working with Medi-Cal/Medicaid or historically underserved populations. Strong communication, administrative, or organizational capabilities. PREFERRED QUALIFICATIONS: Bachelors degree in Social Work, Human Services, Psychology, Public Health, Education, or a related field. Professional licensure or certification (LCSW, LMSW, ASW, RN, CCM). Prior experience with CalAIM ECM, CenCal Health, pediatric care coordination, foster care, or child welfare systems. Deep knowledge of local health and social resources within Santa Barbara or SLO County. Compensation & Benefits: Pay Range: $25.00 $35.00 per hour. Final compensation is determined based on factors such as the candidate's relevant experience, qualifications, skills, and internal equity considerations. Travel & Expense Reimbursement: Eligible field travel, member outreach driving, parking fees, and tolls are fully compensated and reimbursed at the standard IRS business mileage rate (in compliance with California Labor Code 2802).
