Role Overview Registered Nurse - Case Manager at Luke AFB in Glendale, Arizona. Responsibilities Participate in all phases of the Case Management Program (CMP) and ensure that the CMP meets established case management (CM) standards of care. Provide nursing expertise about the CM process, including assessment, planning, implementation, coordination, and monitoring. Identify opportunities for CM and integrate local CM processes. Develop and implement local strategies using inpatient, outpatient, onsite, and telephonic CM. Develop and implement tools to support case management, such as those used for patient identification and patient assessment, clinical practice guidelines, algorithms, CM software, and databases for community resources. Integrate CM and utilization management (UM) and integrate nursing case management with social work case management. Maintain liaison with appropriate community agencies and organizations and accurately collect and document patient care data. Develop treatment plans including preventive, therapeutic, rehabilitative, psychosocial, and clinical interventions to ensure continuity of care toward the goal of optimal wellness. Establish mechanisms to ensure proper implementation of patient treatment plans and follow‑up post‑discharge in ambulatory and community health care settings. Provide appropriate health care instruction to patients and/or caregivers based on identified learning needs. Schedule Monday – Friday, 9 hours between 7:00 am and 5:30 pm, with a one‑hour lunch. No weekends or holidays. Minimum Qualifications Degree: Associate’s or Bachelor’s degree in Nursing from a college or university accredited by the Accreditation Commission for Education in Nursing (ACEN) or the Commission on Collegiate Nursing Education (CCNE). Experience: Minimum of 3 years for ADN or 2 years for BSN full‑time experience in clinical case management with adults, children, families, seniors, and groups. Certification: Certified Case Manager by the Commission for Case Management (CCM) or the American Nurses Credentialing Center (ANCC). Alternatively, Certification Eligible with 3 years of experience for ADN or 2 years’ experience for BSN in full‑time clinical case management with children, families, seniors, or groups. Licensure: Current, full, active and unrestricted license as a Registered Nurse. Life Support Certification: Current AHA or ARC BLS Healthcare Provider certification. Additional Required Skills and Knowledge Assessment: Identify patients for case management; collect comprehensive patient information and medical status; continue evaluating an established plan of care. Planning: Collaborate with the patient, family/caregiver, primary provider, and other health care team members to develop an effective plan of care. Facilitation: Coordinate care and communicate among all involved parties. Advocacy: Support the patient and family/caregivers to ensure identified education and appropriate, timely care is received. Knowledge of medical privacy and confidentiality (HIPAA); accreditation standards of the Accreditation Association for Ambulatory Health Care (AAAHC) and The Joint Commission (TJC); and computer applications/software including Microsoft Office, MS Outlook, and internet familiarity. Communication skills to interact effectively with people who may be physically or mentally ill, uncooperative, fearful, emotionally distraught, and occasionally dangerous. Organization, problem‑solving, and communication skills to articulate medical requirements to patients, families/caregivers, and medical and non‑medical staff professionally and courteously.
