Job Summary: Provide total clinical nursing care for critically ill children during transportation. Communicating and coordinating care with multidisciplinary team members from sending and receiving facilities. Under general supervision, the Registered Nurse II provides individualized, goal-directed patient care to families and patients at a competent level utilizing the principles and practices of the nursing process; delivers safe and effective care and interacts with other members of the health care team to achieve desired results.

Job Description

Scheduled Work Hours/Shift: Rotating 12 or 24 hour shifts, 2-3 days per week, including nights and weekends.

Job Purpose: Provides professional nursing care for patients in the clinical setting. Provides leadership to nursing staff members and collaborates with other members of the health care team to assure quality care for the patients. Promotes, plans, directs, and implements medical management and safe transportation for critically ill/injured infant or child, in consultation with the attending physician. Maintains standards of professional nursing practice. Assesses, plans, implements and evaluates delivery of nursing care in accordance with the medical plan of care, nursing practice standards, nursing care standards, and policy and procedures to achieve optimum patient outcomes for patients of all ages. Collaborates with other transport team members to ensure the safety of all transport operations. Provides rapid assessment, emergency measures and transportation for the critically ill/injured , neonate, infant or child, utilizing the latest technique and state-of-the-art equipment. Acquires and maintains advanced neonatal skills such as intubation, ventilation, insertion of UAC/UVC’s, and chest needle aspirations. Maintains all required certifications, licenses, and continuing education. Organizes, prioritizes, directs, supervises and evaluates nursing care delivered by various levels of staff both in hospital and on transport. Participates in providing outreach education. Maintains pediatric transport quality data. Functions as patient advocate, protector of patient rights and provider of psychosocial support. Frequent direct contact and communication within and outside the MUSC with patients, families, physicians, nurses, EMS personnel, aviation personnel, administrators, and the public. Some communication and coordination with community agencies. Transport work is performed under high tension and sometimes adverse conditions due to the critical nature and location of the patients. Moderate physical effort is required for frequent moving and lifting of transport equipment. Requires substantial walking and standing. Frequently uses mechanical devices. High risk of danger involved in working in and around an aircraft and on an ambulance. Physical requirements include weight restrictions, good general health, and high tolerance to stress.

Minimum Training and Education : Graduation from an accredited school of nursing and a minimum of 5 years of pediatric critical experience in NICU, PICU, PCICU or equivalent. Bachelor of Nursing degree preferred. RN staff hired on or after July 1, 2013, with an Associate or Diploma degree in nursing are required to be enrolled in an accredited BSN program within two years and successfully obtain a BSN degree within four years of the RN hire or reclassification date.  Refer to policy A141 for more details.

Required Licensure, Certifications, Registrations :  Licensure as a registered nurse by the South Carolina Board of Nursing or a compact state. Current Basic Life Support (BLS) required.

If selected, candidate must maintain current BLS, ACLS, PALS, NRP and complete an advanced trauma course (ex: PATC or TNCC) within 1 year of orientation.

Job Responsibilities :

25% - Manage comprehensive nursing care for patients (within specialty area) with needs ranging from uncomplicated to complex and rapidly changing.

Success Criteria:

Based on holistic assessment, identify relevant aspects of the situation and determine the appropriate course of action to move patient/family and staff towards the expected outcome(s).  Assess subtle changes in patient status; anticipate problems before they occur and implement measures to minimize risk.  Ensure completion and documentation of care provided to assigned patients.  Safely perform technical skills, including medication administration in area of practice.

20% - Implement age-appropriate interventions based on individual patient needs.

Success Criteria:

Function as a patient/family advocate demonstrating a caring, compassionate approach. Safely perform technical skills in area of practice. Demonstrate knowledge of hospital policies, procedures, nursing standards, and legal issues in areas of specialty. Coordinate and manage care by applying analytical reasoning, reflection, rational problem-solving skills, using evidence-based information and clinical judgment. Participate in shared decision making and problem solving to promote effective collaboration with interdisciplinary partners.

20% - Organize and prioritize nursing care plans considering the needs of the patients and other staff members.

Success Criteria:

Formulate individualized plan of care utilizing the development, analysis and integration of complex clinical data. Initiate, facilitate and collaborate with the interdisciplinary team to improve care delivered to patients. Utilize the nursing process to facilitate the individualized plan of care using time and resources efficiently effectively.  Design, implement and evaluate a comprehensive, culturally sensitive, patient/family plan of care. Delegate appropriately utilizing professional judgment.

10% - Utilize established outcome measures to evaluate effectiveness of care.

Success Criteria:

Collaborate with sending and receiving care teams and MCO (Med Control Officer) to formulate a safe clinical plan of care Collaborate with the Case Managers to assist with patient throughput and interfacility transport needs Manage resources to promote and target the best clinical outcomes for the specific patient population.

10% - Demonstrate a commitment to quality patient care, environment, and professional development. Success Criteria:a.Identify learning needs and the resources to attain them.b.Serve as a resource to improve patient care and team operations.c.Assist in sharing and teaching best practices and skills to other team members, students and orienteesd.Orient and train new staff, providing timely written and verbal feedback.e.Facilitate a positive learning environment for new hires and provide individualized teaching instructions as appropriate.f.Develop personal objectives and evaluate practice through discussion with others and self- evaluation.g.Demonstrate insight and self-awareness necessary to recognize one’s impact on others and change behavior accordingly. h.Accept and utilize constructive feedback to improve performance.i.Demonstrate effective use of computer-based information for decision support, continuous quality improvement and professional development.

10% - Actively and effectively function in a leadership role as directed.

Success Criteria:

Actively assist others to resolve ongoing problems and unit issues. Develop and implement recommendations to improve the unit environment. Demonstrate responsible, effective representation on a committee, task force, or unit council. Evaluate and develop recommendations for new or revised standards of nursing care and practice in response to patient or unit needs. Effectively manage conflict by confronting the issue and collaborating with others to explore and implement acceptable solutions. Effectively perform in an identified practice focus by providing written evaluation of projects. Demonstrate fiscal accountability while providing quality care.

5% - Participate in research studies and performance improvement initiatives to improve patient care and/or organizational processes.

Success Criteria:

Demonstrate an awareness of current trends and issues in area of specialty through discussion or application in practice. Review and analyze current nursing literature and research in area of specialty to improve practice. Actively participate in unit performance improvement. Conduct an ongoing and systematic evaluation of care given to patients. Participate in research studies available. Collect data for evaluation to improve care.

(I) Ability to lift/lower objects 50 lbs. from/to floor from/to 36 inches unassisted. (I) Ability to lift from 36” to overhead 25 lbs.  (I) Ability to exert up to 50 lbs. of force.  (F) Examples include: To transfer a 100 lb. patient that can not assist in the transfer requires 50 lbs. of force. For every 100 additional pounds, assistance will be required from another healthcare worker. 20 lbs. of force is needed to push a 400 lb. patient in a wheelchair on carpet. 25 lbs. of force is required to push a stretcher with a patient with one hand. Ability to maintain vision 20/40 corrected. (C) Ability to see and recognize objects close at hand or at a distance. (C) Ability to match or discriminate between colors. (C) Ability to determine distance/relationship between objects; depth perception. (C) Good peripheral vision capabilities. (C) Ability to maintain hearing acuity, with correction. (C) Ability to perform gross motor functions with frequent fine motor movements. (C) Ability to deal effectively with stressful situations. (C) Ability to work rotating shifts.
