As one of the largest, free-standing long-term acute care hospitals in the country, our team is dedicated to making a difference; one patient, one family, and one day at a time.**Position Summary:****New Graduate Nurses** are supported throughout their transition from academia to practice with a 12-month Vizient/AACN Nurse Residency Program TM. There is automatic enrollment into the Nurse Residency Program for all Graduate Nurses with less than one year of experience. During the 12-month residency program, Nurse Residents will gather monthly for mandatory seminars to develop clinical and leadership skills. Nurse Residents will take part in a comprehensive orientation and will receive ongoing support and mentorship.Our dedicated nursing team make a lasting difference in the lives of patients with complex, long-term medical needs. Opportunities are available in the following specialty areas: **Acute Care, Continuing Care, Behavioral Health, Pediatrics and the HFSC Hartford location.****Key Responsibilities:*** Provide compassionate, evidence-based nursing care to patients in a long-term acute care or rehabilitation setting.* Collaborate with physicians, therapists, and other care team members to develop and implement individualized care plans.* Monitor, assess, and document patient progress while ensuring safety and comfort.* Educate patients and families on treatment plans, medication management, and discharge planning.* Support a culture of excellence, teamwork, and continuous learning.**Qualifications:*** Required: Successful completion of an accredited school of nursing program.
