Schedule

Shift: 8 AM - 5 PM with weekend rotation options available.

Job Description

We are seeking a dedicated and skilled Registered Nurse (RN) for a home care position. The successful candidate will manage a caseload of 20-25 patients with an expectation of completing 6 home visits per day. The role requires strong communication skills, a team-oriented approach, and the ability to adapt to changing agency needs.

Responsibilities

• Conduct home visits to assess patient conditions and needs.
• Manage care plans and coordinate with interdisciplinary teams.
• Perform clinical assessments and administer medications as prescribed.
• Provide education and support to patients and their families.
• Document patient information accurately in the electronic medical record (EMR) system.

Required Experience / Certifications / Licensure

• Minimum of 2 years of nursing experience, preferably in home health.
• Active RN license in the state of Maine.
• BLS certification.
• Experience with Epic and OASIS documentation required.
• Preferred skills include IV therapy, tube feeds, trach care, wound care, and other advanced nursing procedures.
