PHYSICAL THERAPIST – FULL TIME

Bella Vita Health & Rehabilitation in Glendale, AZ, is a post‑acute care facility that provides long‑term care, short‑term care, and geriatric outpatient services. We are looking for a Full‑Time Physical Therapist to join our in‑house therapy team. You will work in a supportive environment that values excellence, leadership, and professional growth. Full Time – Flexible schedule with work‑life balance. Pay – $42–$50 per hour based on experience. Key Responsibilities Screen, evaluate, and develop appropriate plans of care for patients with cognitive and/or ADL functional disorders following regulatory and clinical standards. Utilize standardized assessments and evidence‑based practice to guide clinical interventions. Provide rehabilitative, skilled, and medically necessary treatment interventions, including ADL retraining, therapeutic activities, exercises, modalities, and the use of supportive and assistive devices. Asses patient progress and adjust treatments accordingly in the clinical record. Evaluate treatment outcomes for effectiveness and modify services or make referrals as needed. Provide consultation and counseling to patients, families, caregivers, and other service providers about physical disorders. Communicate effectively with supervisors and health‑team members regarding patient progress, barriers, and treatment plans. Prepare comprehensive discharge summaries of services provided per regulatory requirements. Qualifications Licensed Occupational Therapist (licensed PT preferred).
