We'd love to meet you!Care Dimensions Healthcare is currently seeking a compassionate, community-centered, and motivated Full-Time Community Health Worker (CHW) to join our growing team. This position will report to our Corona office and primarily support patients throughout the Hemet area.This is a full-time Monday through Friday position with an hourly pay range of $25-$30 per hour, depending on experience.At Care Dimensions, we are committed to providing holistic, person-centered care that supports the medical, behavioral, and social needs of our patients and families. We are looking for someone who is passionate about building relationships, supporting communities, and helping individuals navigate complex healthcare systems with dignity and compassion.Position OverviewUnder the supervision of the Program Lead, the Community Health Worker plays an important role as part of our multidisciplinary care team. The CHW works closely with patients, families, providers, and community partners to support whole-person health and improve access to care and resources.This role includes a combination of outreach, engagement, care coordination, patient support, and field-based community work as part of our Enhanced Care Management (ECM) services.Key ResponsibilitiesSupport patient care through outreach, follow-up, assessments, scheduling coordination, and quality metric initiativesEngage patients and families in navigating healthcare systems and overcoming barriers to medical, behavioral health, and social servicesCollaborate closely with the care team to support a holistic, patient-centered approach to careConduct field-based outreach and maintain ongoing communication with patients and community partnersConnect patients to community resources, social services, health education materials, and supportive programsProvide advocacy, case management support, and compassionate guidance throughout the care journeyAssist with documentation and coordination activities as neededSupport additional program and patient care activities assigned by leadershipQualificationsWe welcome candidates who are passionate about serving their communities and supporting vulnerable populations.Preferred qualifications include:Community Health Worker (CHW) CertificationOR a minimum of 2 years of experience as a CHW, Promotora, Health Navigator, or in community outreach, customer service, medical, behavioral health, or substance use disorder settingsCurrent and valid California Driver's LicenseComfortable traveling throughout Riverside County to support patients and community eventsStrong communication, relationship-building, and organizational skillsAbility to work independently while also collaborating effectively within a team environmentExperience using electronic medical records and computer systemsSpanish-speaking candidates are encouraged to applyAt Care Dimensions Healthcare, we deeply value compassion, teamwork, cultural humility, and community connection.
