The RN provides screening and monitoring of key health indicators and health risk, clinical monitoring for adverse effects of medications, and coordination of Medication Assisted Treatment (MAT). The RN serves as a consultant to other team members and may take over medical care coordination when conditions become complex, supporting recovery with evidence-based and trauma-informed, person-centered care. Responsibilities Provide initial health assessments for all new clinic clients and annual follow-up assessments Coordinate care for medical issues, including all post-admission testing (hepatitis, HIV, tuberculosis, STDs), arranging physical examinations, scheduling appointments, and liaising with providers to obtain records Act as Health Coordinator, providing health education, risk reduction counseling, and referral services for communicable diseases Coordinate and monitor usage of prescribed MAT therapies and psychiatric medications, including lab result review and faxed prescriptions Administer antipsychotic medications to clients as needed Administer intramuscular injections (IM) per Rx orders documented in EHR Collaborate with prescriber on medication refills Conduct toxicology testing as needed Obtain and document lab results from external providers Participate in clinic team meetings and administer injections as prescribed Maintain supplies for the med. room and communicate with pharmacies Apply evidence-based models and theories for substance use and co-occurring disorders, psychoeducation, motivational interviewing, and recovery Provide psychoeducation on medical issues to all clients, individually and in groups Follow agency policies and maintain professional standards Complete all required documentation on time and accurately, including progress notes, health screenings, and care plans Ensure client engagement in health and wellness practices and chronic disease management to reduce hospitalizations and improve quality of life Adhere to all standards and guidelines established by regulatory authorities Other related duties Competencies Experience with evidence-based models for psychoeducation, motivational interviewing, substance use disorders, serious mental illness, and recovery Knowledge of family systems across diverse cultural and ethnic backgrounds Ability to perform duties independently and efficiently Familiarity with and ability to use an Electronic Health Record system Practice in a recovery-oriented, person-centered manner Key Performance Indicators (KPIs) Adherence to treatment protocols and guidelines Assessment of patient understanding and application of health education provided Accuracy and timeliness of client record updates Measurement of patient satisfaction with nursing care and education received Requirements Registered Nurse degree and active New York State license Reliability and ability to work independently, with strict confidentiality Basic computer skills and documentation in an electronic health record Maintenance of required pre-employment clearances Pre-employment drug screen required Legal eligibility to work in the United States without sponsorship On-site 5 days a week, Monday‑Friday 9 am‑5 pm, with one late day 11 pm‑7 pm Special Considerations Hours per week, reporting structure, and location are subject to change per program needs Required coverage of two clinics located in the same building Weekends and on‑call coverage are not required Mandated Reporter This is a Mandated Reporter position. The NYS Justice Center legislation defines anyone who has regular and substantial contact with any person with special needs as a “custodian,” making the RN a mandated reporter.
