Job Summary The Clinical Pharmacist is a licensed professional responsible for providing safe and appropriate pharmacy services to Option Care patients in accordance with the policies and procedures of the Option Care pharmacy practice professional standards and applicable regulatory entities. This entry‑level position introduces the practice of clinical pharmacy in the alternate setting and coordinates patient services with the Nursing Department, monitors patient progress, and educates patients, caregivers, staff and other professionals regarding appropriate medication use. Responsibilities Determine the suitability of individual patients for home care. Collect and organize all patient‑specific information needed to evaluate suitability for home care, medical condition and prescribed medication therapy, and prognosis with clearly defined outcome goals. Design, recommend, monitor and evaluate patient‑specific pharmacy care plans for home care patients; make suitable recommendations for modifications in prescribed medication therapy or monitoring parameters. Collect and organize patient‑specific information to prevent, detect and resolve medication‑related problems and make appropriate medication therapy recommendations. Identify appropriate monitoring parameters for each patient and complete nutritional assessment, making appropriate recommendations. Modify pharmacy care plans for home care patients based on evaluation of monitoring data and other pertinent patient‑specific information. Apply pharmacokinetic principles and formulas and assess patient laboratory values. Provide medication‑use education to Option Care staff, other professionals, home care patients and/or their caregivers; use effective patient education techniques for counseling. Ensure continuity of pharmaceutical care to and from the home and other patient‑care settings; use a systematic procedure to communicate pertinent patient information to and from the home and other patient‑care settings. Prepare and dispense medications using appropriate techniques and following Option Care policies and procedures; calibrate equipment, prepare medications at appropriate concentration, check for incompatibilities, and store appropriately. Adhere to safety and quality assurance practices; prepare labels that conform to organizational policies and procedures and include necessary ancillary labels; inspect final medication before dispensing. Comply with state/federal laws and USP standards, including DEA regulations, perpetual inventory system policies for Schedule II narcotics and other controlled substances. Manage, maintain and troubleshoot medication administration equipment and medication‑related equipment; provide instruction to others on use and maintenance. Participate in assessing, managing and reporting medication‑related incidents, equipment error and/or failures; complete QAR documents according to Option Care policy. Monitor and report discrepancies in perpetual inventory systems for Schedule II narcotics and other controlled substances subject to diversion. Provide concise, applicable, and timely response to requests for drug information from health care providers and home care patients; formulate responses based on literature analysis. Attend educational seminars or participate in other CEU opportunities to promote professional competency and comply with state mandated CEU requirements. Manage the use of investigational drug products according to established regulations and Option Care policies and procedures. Utilize pharmacy support personnel effectively; prioritize workload, organize workflow, and ensure accuracy of work performed by pharmacy support personnel. Comply with accreditation, legal, regulatory and safety requirements; maintain confidentiality of patient and proprietary information; observe legal and ethical guidelines for safeguarding confidentiality. Qualifications Graduate of an accredited School of Pharmacy. Current, active Pharmacy license as granted by the State Board of Pharmacy. At least one year of experience in sterile IV admixture procedures, knowledge of non‑sterile compounding, immunizations and medication therapy management (MTM), completion of an ASHP accredited Pharmacy Residency Program, or completion of an Advanced Pharmacy Practice Experience associated with an accredited school of pharmacy, or equivalent experience approved by the Vice President of Pharmacy. Experience providing customer service to internal and external customers, meeting quality standards for services, and evaluating customer satisfaction. Basic skill in Microsoft Word, Excel and PowerPoint (opening documents/workbooks, formatting, inserting content, basic navigation). Willingness to travel up to 5 % of the time for business purposes within state. Preferred: Completion of an ASHP accredited Pharmacy Residency Program or an Advanced Pharmacy Practice Experience; experience in home care, hospital or clean room setting; at least one year of indirect team management experience.
