ROLE OVERVIEW and PURPOSE:
The Comprehensive Perinatal Health Worker (CPHW) supports the mission and vision of Neighborhood Healthcare (NHC) by providing direct education and support services to prenatal patients. This role is responsible for promoting positive patient experiences, supporting efficient patient flow, and delivering exceptional customer service to all patients throughout their prenatal care journey.
The CPHW ensures that services are provided in accordance with Comprehensive Perinatal Services Program (CPSP) guidelines, NHC policies and procedures, and established program standards. Through patient education, care coordination, and resource linkage, the CPHW helps support positive maternal and infant health outcomes while delivering high-quality, patient-centered care.
RESPONSIBILITIES:
- Conduct CPSP orientation and explain available prenatal services to eligible patients
- Complete CPSP assessments, including Health Education, Nutrition, and Psychosocial assessments, according to program timelines, guidelines, and internal workflows
- Contribute to positive patient health outcomes by working collaboratively with interdisciplinary care teams and supporting continuity of care throughout the prenatal and postpartum periods
- Develop and document individualized care plans based on identified patient strengths, needs, risks, and goals
- Provide health education and counseling on topics related to pregnancy, childbirth, postpartum care, infant care, breastfeeding, car seat safety, disability form support, and family planning
- Coordinate and facilitate referrals to internal and external community resources based on patient needs and follow up on referral outcomes
- Assist eligible patients with presumptive eligibility (PE) enrollment and facilitate referrals for other healthcare coverage resources as appropriate
- Prepare intake packets and provide patients with appointment information, including prenatal, CPSP, laboratory, and specialty care appointments
- Conduct outreach and follow-up for missed appointments, no-shows, and patients at risk of disengaging from prenatal care
- Maintain accurate, timely, and complete documentation in the electronic health record and other required program systems
- Monitor assigned patient panel to ensure completion of required CPSP services, assessments, referrals, education, and follow-up appointments
- Collaborate with prenatal care providers, care teams, and community partners to support coordinated, patient-centered care
- Participate in CPSP-required trainings, staff meetings, quality improvement activities, and continuing education opportunities
- Assist with data collection, reporting requirements, chart reviews, and other responsibilities necessary to support CPSP compliance and program performance
- Support implementation of CPSP policies, procedures, and best practices to ensure standardized service delivery across Neighborhood Healthcare
- Promote patient engagement and retention in prenatal care and CPSP services through outreach, education, and ongoing support
- Provide culturally responsive, trauma-informed, and patient-centered services that respect the diverse backgrounds and needs of patients and families
- Demonstrate values in action in all patients and team interactions
- Operate in a manner that instills confidence and trust in Neighborhood Healthcare, its programs, and its commitment to high-quality care
- Provides Telehealth and/or onsite coverage between all health center locations as needed to ensure continuity of CPSP services and patient access to care
Travel Responsibilities
- Must be willing and able to float between Neighborhood Healthcare locations based on business and patient care needs
- Reliable transportation may be required to travel between assigned clinic sites
EDUCATION/EXPERIENCE:

High school diploma/GED required ; associate's degree preferred

One-year perinatal or prenatal care experience within community health centers required

CPSP Provider Training and/or Medical Assistant certifications preferred

Bilingual (English/Spanish) highly preferred

Knowledge of prenatal, postpartum, and perinatal health education principles and community resources

Knowledge of CPSP guidelines, documentation requirements, and patient-centered care practices

Ability to maintain accurate and timely documentation in electronic health records and program systems

Strong organizational skills with the ability to manage multiple priorities and meet required deadlines

Ability to assess patient needs and coordinate appropriate referrals and follow-up services

Ability to work independently and collaboratively as part of an interdisciplinary team

Ability to handle sensitive and confidential information with professionalism and discretion

Cultural sensitivity and the ability to provide compassionate, trauma-informed care to diverse patient populations
Physical Requirements

Ability to lift and/or carry up to 25 pounds

Ability to sit for prolonged periods of time and perform computer-based work

Ability to move throughout the clinic as needed to assist patients and collaborate with care team members
Pay Range: $27.21 - $38.09 per hour, depending on experience

While this range represents the broader classification of the role, actual compensation will be based on several factors, including but not limited to: the candidate’s overall knowledge, skills, and experience, market data and industry benchmarks, internal equity within the organization, Budgetary considerations and organizational needs. As a result, placement within the range is not guaranteed, and the full pay grade range may not be utilized.
