Key Responsibilities

• Conduct thorough psychosocial assessments to identify the mental health needs, strengths, and resources of hospice clients and their families.
• Deliver individualized, family-oriented, and group therapy sessions employing evidence-based practices such as cognitive-behavioral therapy, solution-focused therapy, and trauma-informed care.
• Develop, implement, and continually assess personalized treatment plans with measurable goals, ensuring alignment with clients' changing needs.
• Collaborate seamlessly with interdisciplinary teams—including psychiatrists, psychologists, and medical staff—to coordinate comprehensive care strategies tailored to holistic wellness.
• Provide crisis intervention and safety planning while addressing immediate emotional and psychological needs as they arise.
• Facilitate workshops and support groups that promote psychoeducation, emotional expression, and community support among clients and their families.
• Maintain meticulous documentation of client interactions per ethical and legal standards, using our integrated software systems to support continuity of care.
• Engage in regular case consultations with fellow clinicians to enhance treatment approaches based on shared experiences and client progress.

Patient Population / Client Focus

This position primarily focuses on supporting individuals and families navigating the complexities of hospice care. Our clientele includes patients diagnosed with terminal illnesses, as well as their loved ones who require guidance and support through this profound life transition. Sensitivity, empathy, and the ability to facilitate conversations around grief, acceptance, and loss are essential traits for success in this role.

Qualifications

• A Master’s degree in Social Work from an accredited institution.
• Active, unrestricted state licensure as a Licensed Clinical Social Worker (LCSW).
• Demonstrated expertise in mental health counseling with at least two years of practical experience in a mental health or outpatient setting.
• Proficient understanding of evidence-based therapeutic modalities and compassionate care frameworks.

Compensation and Benefits

The annual salary for this position ranges from USD 80,000 to 100,000, commensurate with experience, plus a sign-on bonus.
