RN Case Manager, Cardiology - Las Vegas, NV

Role Overview The RN Case Manager - Cardiology is responsible for care coordination and management of high-risk cardiac and CHF patients, both inpatient and outpatient. This position involves collaborating with providers, supporting remote telemonitoring, developing individualized care plans, patient education, and ensuring smooth transitions of care to improve clinical outcomes. Key Responsibilities Apply case management standards to high-need patients Act as a patient advocate and resource for the care team Develop and monitor personalized care plans with patients and providers Assist with remote monitoring and telehealth initiatives Educate patients on disease management and lifestyle modifications Maintain accurate records, analyze data, and report outcomes Coordinate care during transitions and reduce readmission rates Perform clinical assessments and provide telephonic nursing support Utilize clinical software and demonstrate strong communication skills Qualifications & Skills Active RN license in Nevada and BLS certification Minimum 3 years of critical care nursing experience Knowledge of case management, UM, and plan benefits Strong clinical assessment, critical thinking, and communication skills Proficiency with MS Office and practice management systems Preferred Qualifications Bachelor’s degree in healthcare or related field CCM and ACLS certifications Experience in cardiology and case management Work Environment & Compensation Normal clinic setting with travel within the local area.
