Remote IRIS Consultant: Empower Care & Goals

Molina Healthcare of Illinois

Molina Healthcare is hiring an IRIS Consultant (IC) to support Wisconsin IRIS program participants from a home-based setup with regular in-person visits. You will help individuals define personal goals in employment, housing, health, and safety and coordinate with local resources, while maintaining HIPAA compliance. You will document activities, educate participants on budgets, and collaborate with IRIS staff to ensure successful plan implementation across diverse communities. #J-18808-Ljbffr
